Once You're In Your Records, Watch For This.
Open your portal in another tab if you can. Skim for these specific things — they don't prove anything on their own, but they're worth a second look.
Records that look different from what was communicated verbally at the time
Medication changes with no documented explanation — especially sedation, opioids, or anything affecting consciousness
Notes describing family being asked to leave during conversations about care
A records request met with delay, resistance, or legal threats instead of the records themselves
A condition that worsened rapidly with no clear clinical explanation in the chart
A visit, procedure, or conversation documented that you don't remember happening
A billed date of service that doesn't match when care actually happened

This is the only part of this that actually matters for the data we're gathering. Answer honestly, even if the answer is "no" or "I'm not sure."
